The route begins in Ciechanów, and then runs near such towns as Plonsk, Wyszogrod, Sochaczew, Zyrardow, Mszczonow, Grojec, Góra Kalwaria, Kolbiel, Minsk Mazowiecki, Stanislawow and Lochow, ending on the bypass of Ostrów Mazowiecka.
In the section between Wyszogrod and Lochow, national road No. 50 is part of the so-called "Large Bypass of Warsaw".
